The initial stage of treatment for periodontal disease is usually a thorough cleaning that may include scaling or root planing. The objective of these non-surgical procedures is to remove etiologic agents such as dental plaque and tartar, or calculus, which cause gingival inflammation and disease.

Scaling and root planing can be used as a stand-alone treatment, or a preventative measure. They are commonly preformed on cases of gingivitis and moderate to severe periodontal disease.

What do the procedures entail?

Dr. Marcus or Dr. Tanna will only perform scaling and root planing after a thorough examination of the mouth, which may include taking x-rays and visually examining the mouth. Depending on the condition of the gums, the amount of tartar present, the depth of the pockets, and the progression of periodontitis, Drs. Marcus or Tanna may recommend scaling and root planing.

In some cases, a local anesthesia may be used during the procedure.

  • Scaling:
    When scaling is performed, calculus and plaque that attaches to the tooth surfaces is removed. The process especially targets the area below the gum line, along the root. Scaling is performed with a special dental tool called an ultrasonic scaling tool. The scaling tool usually includes an irrigation process that can be used to deliver an antimicrobial agent below the gums to help reduce oral bacteria.
  • Root Planing:
    Root planing is performed in order to remove cementum and surface dentin that is embedded with unwanted microorganisms, toxins and tartar. The root of the tooth is literally smoothed, which promotes healing, and also helps prevent bacteria from easily colonizing in the future.

Antibiotics or irrigation with anti-microbials (chemical agents or mouth rinses) may be recommended to help control the growth of bacteria that create toxins and cause periodontitis. In some cases, Drs. Marcus or Tanna may place antibiotic fibers in the periodontal pockets after scaling and planing. This may be done to control infection and to encourage normal healing.

When deep pockets between teeth and gums are present, it is difficult for Drs. Marcus or Tanna to thoroughly remove plaque and tartar. Patients can seldom, if ever, keep these pockets clean and free of plaque. Consequently, surgery may be needed to restore periodontal health.

Benefits of Treatment

If treatment is successful, scaling and planing may have many periodontal benefits. One is that it can help prevent disease. Research has proven that bacteria from periodontal infections can travel through the blood stream and affect other areas of the body, sometimes causing heart and respiratory diseases.

Scaling and root planing remove bacteria that cause these conditions.

Another benefit of treatment is protecting teeth against tooth loss. When gum pockets exceed 3mm in depth, the risk for periodontal disease increases.

As pockets deepen, more bacteria are able to colonize, eventually causing a chronic inflammatory response by the body to destroy gingival and bone tissue. This leads to tooth loss.

Finally, scaling and root planing may make the mouth more aesthetically pleasing, and should reduce bad breath caused from food particles and bacteria in the oral cavity.

Superficial stains on the teeth will be removed during scaling and planing, adding an extra bonus to the procedures.

Dr. Marcus examined him and determined that he needed a root canal and referred us to an endodonist. He didn’t charge us because of his kindness and concern for our situation. Dr. Marcus has done lots of major work on me over the decades: bone grafts, implants, and lots of maintenance. I had seen another periodontist who told me I would be lucky to have of my original teeth after ten years. That was forty years ago. My regular dentist tells me that the implants that are done by Dr. Marcus are as straight and well-positioned as possible. He is considerate about patient pain and discomfort. Also a heck of a nice guy. If you need a periodontist, this is the best.

I felt no pain during and even after a very extensive procedure which I attribute to his knowledge and expertise. I need a look at my Gums as per my Dentist.. Dr Marcus was Right on it... He took two Samples..I felt Nothing.. Gave me tips and How To's... also his Cell Number.... He text me that night all was well.. Then the next night I had a Problem.. Bleeding I texted him he call told me to. try a Tea Bag !! and it worked But, did Not stop all the Flow... He Called me at 10:20 pm.. Asked how it was going I said not stopping yet... He said I will meet you at my office NOW... He Worked on me and tried twice to Cauterize the Flow.. 80 % stopped... He said he needed to do something else and he explained the Stitching of my Cheek skin over the wound... It Worked... He Finished me up and we left his Office about 1 am... He then Called me the next Day.. and Sid if ant problems get in touch with him..... Thank You Very Much... His kind of Service is VERY HARD to Find today.... and I will tell anyone who will listen How AWESOME he is with his WORK....

If Marcus recommends laser surgery make sure you ask him about the success rate. Meaning typically how much bone growth is expected. I think laser surgery is basically a scam for periodontists. It's expensive and painful and will not yield or save your teeth from bone loss. Be sure to ask "How many teeth have you saved?"

If I could rate Dr. Marcus higher than a 5 I would, he is amazing. I was in to have an implant and sinus lift not too long ago. I was so nervous about the procedure, the nurses reassured me that I was in good hands and that I had nothing to worry about. They were right, it was quick and painless. Over the course of the last 14 months I had a lot of Implant work done with Dr. Marcus. One visit I had 2 implants put in my lower jaw and after they healed I had 4 implants put in the upper jaw. They have all healed now and I am very pleased with the result. It was a big decision I made and I have to admit I was worried about how much pain I was getting myself in for, but I can honestly say it was not that bad. The two implant surgerys were painless. There was some soreness and a little swelling the evening of the surgery and Dr. Marcus called me at home to see how I was doing. I would highly recommend Floral Vale Periodontics and Implants.

I'm reluctant going to the dentist especially for something complex. I needed implants with bone grafting which I put off for about three years. I wish I had gone earlier and solved my dental problems sooner because the visit was painless and fairly quick. Also the followup was excellent. Dr. Marcus called to check on my condition the next day and the two later checkup visits were a breeze.

Many may not know that Dr. Marcus is a respected leader in the field of laser surgery and a noted teacher and mentor to dental professionals throughout the country. Here's an inexpert review of his work: Dr. Marcus treated me for periodontitis, a condition resulting from the loss of gum and bone tissue due to those nasty gum pockets which start out as gingivitis, but grow so deep that daily maintenance and regular dental cleanings cannot reverse. Ultimately, loss of bone tissue can lead to loss of teeth. Using the latest laser technology available to dentistry, Dr. Marcus cleared embedded bacteria and re-established the growth of new gum and bone tissue. He also performed invasive gum surgery to correct an unreachable infected root appendage that normal root canal procedures cannot access. So Dr. Marcus is achieving success in the treatment of periodontal disorders that were beyond yesterday's periodontal reach. And he is among a small group of laser surgeons with advanced training and equipment to make it happen.
